About The Role
We are seeking an experienced clinical pharmacist with a passion for delivering best-in-class patient care to join our rotational specialist pharmacy team. Successful candidates will work across key specialties including intensive care, cancer services, respiratory medicine, and elderly care.
Key Responsibilities
- Provide clinical pharmacy services in assigned specialties, including Medicines Reconciliation on admission, prescription reviews, and discharge optimisation.
- Process and manage prescription planning for discharge pathways.
- Participate in pharmacy on-call, late-night, and weekend rotas as required.
- Mentor and support foundation-level and trainee pharmacists in training.
- Collaborate with Advanced Clinical Pharmacists on service development and clinical reviews.
Essential Requirements
- Masters qualification in Pharmacy.
- Registered as a pharmacist with the General Pharmaceutical Council (GPhC).
- Completed (or working towards) a Diploma in Clinical Pharmacy or equivalent advanced practice with supporting training.
- Proven clinical experience across multiple specialties, demonstrating advanced proficiency.
- Strong commitment to patient-centred care and continuous improvement.
