Band 6 Specialist Physiotherapist – Home 1st Service

Join Home 1st interdisciplinary team and help people regain independence where it matters most – at home.

Home 1st is a dynamic community based rehabilitation service focused on promoting independence, preventing unnecessary hospital admissions, and reducing hospital length of stay. We work across a range of settings including patients’ homes, inpatient units, hospitals, and community venues.

You will be part of a supportive multidisciplinary team, working alongside physiotherapists, occupational therapists, nurses, and rehabilitation support workers to deliver high-quality, patient-centred care.

Main Duties of the Job

About The Role

As a B6 Specialist Physiotherapist, you will manage your own defined caseload and deliver evidence-based, personalised rehabilitation programmes. You will work with individuals presenting with a wide range of conditions, including:

  • Post-orthopaedic and general surgery recovery
  • Post-critical care rehabilitation
  • Frailty and falls
  • Stroke and neurological conditions (e.g. MS)
  • COPD and respiratory conditions
  • Dementia
  • Musculoskeletal conditions
  • Palliative care needs

What You’ll Do

  • Assess, plan, and deliver individualised treatment programmes using evidence-based practice
  • Work autonomously while having access to guidance and support from experienced clinicians
  • Contribute to maintaining a high-quality, efficient, and effective therapy service

Detailed Job Description And Main Responsibilities

Band 6 Specialist Physiotherapist - Home 1st Therapy

Role Overview

  • Deliver high-quality, person-centred physiotherapy assessment and rehabilitation within the Home 1st service
  • Work as an autonomous practitioner across community pathways, supporting admission avoidance, early discharge from hospital and community therapy
  • Collaborate with the multidisciplinary team (MDT) and external agencies to deliver integrated care

Key Responsibilities

  • Assess and provide evidence-based treatment plans for a varied patient caseload
  • Support rehabilitation, discharge planning and patient independence
  • Act as a Trusted Assessor where appropriate
  • Maintain accurate clinical records and manage clinical risk
  • Communicate effectively with patients, families and professionals

Professional & Leadership Duties

  • Maintain HCPC registration and adhere to CSP standards
  • Engage in continuous professional development (CPD) and clinical supervision
  • Contribute to service development, audits and quality improvement
  • Supervise and support junior staff and students where required

Working Pattern & Location

  • Full-time (37.5 hours) with a flexible shift pattern including evenings, weekends and bank holidays
  • Based at Bletchley Community Hospital with community-based working
  • Service hours may expand to 8am–8pm

Person Specification

Qualifications

Essential Criteria

  • HCPC registration
  • Degree in Physiotherapy
  • Membership of recognised and relevant professional body
  • Demonstrate/evidence of recent continuing professional development

Desirable Criteria

  • Car driver with access to a car for work

Experience

Essential Criteria

  • Previous substantial experience of working in the NHS
  • Ability to manage own workload, determine priorities and make autonomous decisions
  • Experience post registration in Community/ Acute /Stroke/ Rehabilitation / Frailty settings
