About the Role

North Sefton Planned Therapy service provides therapy and rehabilitation in patients' homes and care home settings. We aim to promote independence and improve quality of life for people with physical problems caused by accidents, ageing, disease, or disability. These include respiratory conditions, orthopaedic and musculoskeletal conditions, mobility issues, neurological conditions, and general age-related decline, where rehabilitation goals are identified.

About You

If you’re motivated, with therapy experience and skills, a passion to be part of this service, and share our aim for perfect care, we’d like to hear from you. We are looking for an individual experienced in community care, who has experience managing patients with multiple and complex long-term conditions.

We are seeking an individual who has experience supervising both staff and students, and looks to develop the workforce. If you are passionate about continuous development and have contributed to service development, we would like you to apply.

The Team

We’re a very supportive team with physiotherapists, occupational therapists, rehab assistants, and assistant practitioners who work closely with other members of the MDT.

The post holder will be expected to effectively manage a caseload of patients with multiple and often complex co-morbidities as an autonomous practitioner, using evidence-based patient-centred principles to assess, plan, implement, and evaluate interventions of the patient group.

The post holder will provide leadership for junior staff, when appropriate, through supervision and appraisal. The post holder will participate in planning, development, and evaluation of services provided, holding responsibility for defined service improvement projects. The post holder must have experience supervising junior staff and students.

The post holder will take a holistic approach in the assessment and treatment of patients, being responsible for the provision of appropriate, high-quality therapy and care to a caseload which may be of a complex nature, using the appropriate model of care and evidenced-based practice.

The post holder should be able to complete a comprehensive assessment, including the appropriate use of outcome measures and generate a goal-focused treatment plan.
