Job Overview
We are seeking a dynamic and enthusiastic Specialty Doctor to work on our Older Adult Home Treatment Team in the Harrow Older Adult Service, Bentley House, Harrow.
The role is to provide clinical input to the Older Adult Home Treatment Team in Harrow. The post is 10PA Permanent and aligned with Harrow OPHA CMHT. The borough provides rich opportunity for audit and research and support for achieving personal development plans.
Main Duties of the Job
The appointed doctor will have a central role in providing medical input within the Harrow OPHA Home Treatment Team. The team will be composed of senior Band 7 nurses along with Band 6 and 5 nursing staff and administrative resource. The doctor will work closely with the other members of the multi-disciplinary team, in order to ensure that patients are provided with the highest standards of care. This will require regular reviews of caseload, management plans and addressing risk. There will be close working with Harrow OPHA CMHT, Memory Service, Liaison Psychiatry, and Ellington Ward.
The doctor will also be required to perform other clinical duties including home visits, Mental Health Act assessments, and reviews in residential homes as required. The post holder will be expected to ensure that any leave taken is covered by local older adult consultants.

All staff receive regular training and supervision in both formulation and treatment, using this approach. The care is delivered by an integrated multi-disciplinary team with a shared sense of ownership for patients.

Detailed Job Description And Main Responsibilities
Clinical Responsibilities
- To holistically assess patients formulating individualised Care Plans with service users and their carers that aims to address their psychological, physical, spiritual & social needs, incorporating treatment and signposting / referring to other services where necessary.
- Provide medical consultation, advice, and expert knowledge to all team members.
- Involvement within normal working hours in urgent assessments (which might involve Mental Health Act assessments)
- Provide support, advice, liaison, and information to all who might be concerned in the care of the patient. This includes education and advice to family members/carers of patients as well as professional carers.
- To formulate risk assessments and ensuring all clients have appropriate care plans in place which addresses the risks.
- To maintain up-to-date clinical records electronically in line with agreed local policies and procedures, guidelines from professional bodies, and CNWL policy.
- To liaise with carers, general practitioners, acute hospital services, social services, and third sector organisations.
- Regular medication review and inform GP’s of any changes
- To ensure all key performance indicators are maintained in line with trust policies.
- To have a good knowledge of safeguarding of vulnerable adults and report to the necessary agencies.
Management and Leadership
- To contribute to the development of a culture of risk assessment and management to be delivered effectively in own area of responsibility and practice
- To represent the team/sector on both internal and external clinical and strategic planning forums and to act in a professional manner at all times
The post-holder will contribute to the supervision of the trainees and is expected to contribute and support in other clinical duties and writing discharge summaries. To work as part of the multi-disciplinary team and to provide expertise within the department, upholding professional standards at all times.

Education And Qualifications
Essential Criteria
- Primary Medical degree
- Full GMC registration
Desirable Criteria
- Section 12 approval
Clinical Experience
Essential Criteria
- Completed at least four years full-time post-graduate training (or its equivalent gained on a part-time or flexible basis) at least two of which will be in a specialty training programme in a relevant specialty or as a fixed term trainee or have equivalent experience and competencies
- Extensive experience in the management of a broad range of relevant mental disorders in adults
Desirable Criteria
- Experience in managing patients from different cultural backgrounds
- Working with interpreters and translation services
Skills and Knowledge
Essential Criteria
- Ability to treat service users with respect and dignity at all times, adopting a culturally sensitive approach
- Able to work effectively in a team
- Able to build constructive relationships
- Ability to plan treatment programmes in conjunction with multi-disciplinary team

- Up to date knowledge of recent DH documentation and NICE Guidelines
- Experience in teaching junior doctors and other professional groups
