Clinical Fellow (Tier 1) in Acute Paediatrics and Neonatology – Wrexham Maelor Hospital
Job Overview
Applications are invited for the post of Clinical Fellow (Tier 1) in Acute Paediatrics and Neonatology, based at the Wrexham Maelor Hospital. As part of a busy and dynamic department, this role offers a mission to deliver high-quality care to local children and young people, with strong collaboration between consultants, specialist paediatric nurses, and tertiary centres like Alder Hey Children’s Hospital, Liverpool.

Department & Facilities

Paediatric Ward:
- ~5,000 annual admissions (2,600 emergency)
- 22 beds, including 1 high-dependency bed and 2 en-suite oncology cubicles
- Assessment Unit operational:
- 0700–2100 (weekdays)
- 0900–1800 (weekends)
- Manages ENT, trauma/orthopaedics, urology, and general surgical cases (elective and emergency)
- Joint care with surgical teams for paediatric emergencies

Neonatal Unit:
- Self-sufficient for premature babies ≥27 weeks gestation
- Transfers infants <27 weeks to higher care

Additional Support:
- Regular teaching sessions
- Joint Radiology meetings
- Proactive coverage of annual leave, study leave, and statutory days
Team Composition
The Paediatric team at Wrexham consists of:
- 10 consultants
- 11 Tier 2 Fellows
- 13 Tier 1 Fellows (total)
Main Responsibilities
Clinical Duty Scale
- Daily & Ongoing Care (ward admissions + outpatient follow-up)
- Consultant-level responsibility for patients, shared across 13 Tier 1 Fellows (2 supporting neonatal unit, 11 guiding paediatric/outpatient duties)
- On-call structure:
- Night duty: 1 Fellow on shift
- Day cover: 2 Fellows on-call
- Regular rotation ensures fair coverage
- Out-of-hours structure:
- 1:8 on-call model (covered)
- Weekday/weekend nights and long shifts
- Shift length: ~12.5 hours (on-call)
- Day shifts: 8 hours
- 1:8 on-call model (covered)

Person Specification
Qualifications (Essential Criteria)
- Fully GMC-registered Medical Practitioner
Desirable Qualifications
- MRCPCH (Parts 1A+1B)
Experience (Essential Criteria)
- minimum 1 year of full-time working experience in:
- General Paediatrics
- Neonatal care
- Relevant background must be clearly documented in applications; assessed during interviews
Desirable Experience
- UK practice experience
- ST-level (post-Foundation) paediatric/neonatal experience
- Completed/remain committed to:
- Advanced Paediatric Life Support (APLS)
- Newborn Life Support (NLS)
- Child Protection Courses
Skills & Abilities (Essential Criteria)
- Clinical competence:
- Examine normal children while rapidly recognising illness signs
- Manage sick paediatric cases without immediate supervision (with safety self-awareness)
- Communication skills: oral and written (e.g., clear report drafting)
- Time management
- Collaborative working across multi-disciplinary teams
Desirable Skills
- Independence: Proven ability to lead decisions unsupervised
- Reporting: Professional writing experience
- How to stand out:
- Motivation to audit, research, or teach

- Understanding of the audit cycle
- Commitment to participate in research/audit activities
- Willingness to tackle teaching (e.g. trainees)
