Senior Staff Nurse – OPS Hospital at Home Service

About the Role

Hospital at Home is seeking a qualified and motivated staff nurse to join our growing OPS (Older Persons) Hospital at Home service, part of the virtual wards specialising in acute community care. The successful candidate must be passionate about delivering excellence in person-centred, holistic care to patients and their families.

The Hospital at Home service aims to prevent unnecessary hospital admissions by caring for patients safely within their own homes or community settings, ensuring they receive clinical support when traditional acute hospital care is not required.

Key Responsibilities

  • Conduct assessments and implement care plans for patients safely managed at home or in community environments.
  • Focus on preventing avoidable hospital admissions through skilled home-based acute care interventions.
  • Work within a multi-disciplinary team to deliver effective, patient-focused care.

Essentials

✅ Valid UK driving licence (essential due to travel requirements) ✅ Current NMC registration ✅ Previous NHS experience (due to the autonomy of the role) ✅ Experience in acute care or community nursing preferred

Locational Details

  • Based in Christchurch, with daily travel required within the BCP locality.
  • Note: If the role moves sites, excess mileage reimbursement may not apply, and travel from home to the new base will be considered standard commuting.
