Role Summary

The Stoma Care Nurse Specialist is responsible for delivering high-quality, evidence-based stoma care services to patients in hospital, community, and home settings. The role supports patients from surgery through long-term management, helping them achieve independence, improve quality of life, and optimise clinical outcomes. Working in partnership with the NHS stoma care service within the Preston area you will work within the acute setting 1 day a week and provide community support for the rest of the week. The successful candidate will act as a trusted clinical expert, providing education, support, and guidance to patients, carers, healthcare professionals, and key stakeholders while maintaining the highest standards of professional and ethical practice.

Your Responsibilities

Clinical Care

  • Provide specialist stoma care assessment, education, and support to patients before and after surgery.
  • Deliver personalised care plans that promote patient independence and self-management.
  • Conduct home visits, virtual consultations, and clinic appointments as required.
  • Assess and manage stoma-related complications including leakage, skin complications, appliance fitting issues, and lifestyle concerns.
  • Support patients in selecting appropriate stoma care solutions based on individual clinical needs.
  • Provide emotional support and counselling to patients and families adapting to life with a stoma.
  • Maintain accurate, timely, and confidential patient records in accordance with regulatory requirements.

Patient Education

  • Educate patients and carers on stoma management, appliance use, skin care, nutrition, hydration, and lifestyle considerations.
  • Develop and deliver educational materials and resources that support patient understanding and adherence.
  • Encourage patient confidence and independence through ongoing coaching and follow-up.

Healthcare Professional Support

  • Build strong working relationships with colorectal surgeons, stoma care nurses, ward teams, district nurses, GPs, and community healthcare professionals.
  • Provide clinical education and training sessions to healthcare professionals.
  • Support continuity of care across acute, community, and primary care settings.
  • Participate in multidisciplinary meetings and patient pathway discussions where appropriate.

Product and Clinical Expertise

  • Maintain expert knowledge of ostomy products, accessories, and emerging innovations.
  • Provide impartial, evidence-based product guidance based on individual patient needs.
  • Support product evaluations and patient trials where clinically appropriate.
  • Ensure all activities comply with relevant clinical, regulatory, and ethical standards.

Your Profile

  • Registered General Nurse (RGN) with active NMC registration.
  • Minimum 3 years post-registration nursing experience.
  • Experience in colorectal, surgical, community, tissue viability, or stoma care nursing.
  • Strong clinical assessment and patient education skills.
  • Excellent communication and relationship-building abilities.
  • Ability to work autonomously and manage a geographically dispersed caseload.
  • Full UK driving licence.
