Support Coordinator

Salary: £27,500 p.a. | Equivalent pay: £14.06 p.h. Leave: 34 days (rising to 39) + health cash plan worth over £1,140 Contract: Permanent, full-time (37.5 hours per week)

Location: Sunderland (community-based, East Coast & Yorkshire region)

Support Coordination is a dynamic role where no two days are the same. One day you might help someone achieve a significant life goal; the next, steady them through everyday challenges. You’ll contribute to welcoming customers into a place they can call home—literally and metaphorically.

What You’ll Do

  • Lead support planning after coordinating multi-agency referrals and allocating resources
  • Oversee support delivery by guiding our Support Workers to achieve personalised, person-centred outcomes for customers
  • Balance and prioritise support, safeguarding documentation, administrative tasks, meetings, and daily managerial responsibilities
  • Coordinate caseloads effectively, ensuring timely and holistic support for customers
  • Assist with everyday tasks such as budgeting, meal preparation, and accompanying customers to appointments or therapy sessions
  • Collaborate with safeguarding teams, community partners, and external agencies to empower independence and safeguard well-being
  • Prepare and support customers for life transitions, such as independent living in their communities

What You’ll Bring

We’re looking for someone who:

  • Has a burning passion for supporting others, ideally in a supported housing setting
  • Understands complex care needs and can take ownership of planning bespoke, inclusive solutions
  • Holds (or is pursuing) a Level 3 Diploma in Care—alternatively, has earned equivalence through sector experience
  • Has confirmed capability in assessing referrals, coordinating teams, and managing caseloads effectively
  • Can demonstrate resilience and initiative: picture calm, confidence, and adaptability under pressure
  • Enjoys problem-solving, sees dependency as a temporary state, and is committed to progressive independence for customers

  • Hours: 9 am–5 pm (Monday–Friday), with potential to adapt to evening/weekend shifts (leading to 8 pm–8 pm weekends; driven by service demand)
  • Location: Community-based across the Sunderland area
  • Certification: Enhanced DBS check clearance (we pay). Must see to drive for business use, with demonstrated eligibility for relevant insurance.
  • Mileage: Unlimited business mileage, with reimbursement provided.
