Support Coordinator (Healthcare and Social Care) – Permanent – Shotton Colliery, Durham
Key Details
- Salary: £27,500 / £14.06 per hour + £1,140+ health cash plan
- Leave: 34 days (pro-rated), rising to 39
- Type: Permanent, full-time (37.5 hours per week)

What You’ll Do
- Lead support planning and delivery: Create and manage person-centred support plans tailored to each individual.
- Balanced responsibility: Juggle support, safeguarding, administration, and meetings while mentoring colleagues and managing your caseload effectively.
- Practical aid for daily living: Assist customers with budgeting, cooking, attending appointments, and other essential activities.
- Collaborative safeguarding: Work closely with multi-agency teams to ensure our customers are empowered and protected.
- Promote independence: Help individuals build resilience so they can thrive outside traditional支援 contexts.

Ideal Candidate Profile
This role is best suited to someone who thrives on purpose and connection:
- Passion for care: Strong desire to support others, ideally in a supported housing or care setting.
- Qualification/competency: Level 3 Diploma in Care (or equivalent experience). Willingness to work toward the qualification if required.
- Experience: History of assessing referrals and coordinating support.
- Character traits: Highly initiative-driven, resilient, and capable of remaining calm under pressure.
- Tech-ready: Basic proficiency in Microsoft Word & Excel; ability to use support planning software, complete online training, and collaborate via digital tools.
- License and safeguarding: A clean Enhanced DBS (paid for by us) and a valid driving license & insured vehicle (tracked mileage reimbursed).

The Practical Details
Kickstart with a 4-week rolling rota, including weekends and bank holidays. The role may occasionally require flexible shifts to accommodate our customers’ needs—though we’ll do our best to balance it for you.
You’ll find lone working during some morning and evening shifts.
Acceptance criteria:
- Vehicle & insurance: Must be privately insured for business use (mileage covered).
- DBS: Apply/authorise your Enhanced DBS clearance quickly.
