28-Hour Support Facilitator – Hawthorn House

Key Details

  • Location: Hawthorn House (res likenfial care home for adults with learning disabilities)
  • Hours: 28 per week (incl morning, afternoon, weekend, and bank holiday duties).
  • Pay: Standard rate plus 10.6% for after-hours/work. Please note: This role is not eligible for sponsorship due to UK Immigration Act 2025 (effective 22 July 2025).
  • Interviews: Week commencing 6 July 2026.

Main Responsibilities

You will support individuals to achieve their full potential by:

  • Personal and intimate care: Ensuring dignity and respect for all service users, including bathing, dressing, and other needs.
  • Dietary support: Adhering to care plans for meal and drink assistance.
  • Manual handling: Assisting individuals safely with mobility and transfers.
  • Pos itve behaviour management: Applying de-escalation and safety interventions to foster a calm environment.
  • Community engagement: Escorting and supporting individuals during outings, errands, and social activities.
  • Person-centred care: Involving service users in planning their own care and developing independence.
  • Activities planning and delivery: Organising and facilitating therapeutic/recreational activities tailored to individual needs.
  • Recording and reporting: Completing accurate digital care records.

What You’ll Bring:

✔ A genuine enthusiasm for working with adults with learning disabilities (including autism). ✔ Compassionate, sensitive, and patient demeanour, prioritising independence. ✔ Strong ICT and digital record-keeping skills for care documentation. ✔ Excellent communication and interpersonal abilities to connect meaningfully. ✔ Ability to diffuse conflict calmly and manage aggression without escalation. ✔ Physical readiness for manual handling, transferring, and supporting personal care. ✔ A proactive attitude towards change and adaptability in a varied environment.
