About the Role

We are looking for a dedicated and person-centred Support Housing Advisor to join our team, identifying the support needs of our residents and empowering them to live independently and well. You will assess needs, create tailored support plans, signpost to the right services and administer financial support where needed, alongside completing housing management tasks across our independent living and supported housing schemes.

We own and manage c.8,000 homes in the east of England, working closely with other agencies and stakeholders within our communities. We build on the good foundations we have in place in being a customer-centric organisation, investing in our existing homes - including our zero-carbon journey - as well as building new homes.

Outline of Key Responsibilities

  • Guide and support residents across all contact channels, forging links with external statutory and voluntary agencies - including Social Services, GPs, mental health and addiction services - to promote independent living and ensure support is accessed when needed
  • Record, report and refer safeguarding concerns including the completion of DASH forms for our residents experiencing domestic abuse, medical and social issues, liaising with external agencies and attending multi-agency meetings to ensure a joined-up approach to resident support
  • Complete and review support plans, PEEPs and person-centred fire risk assessments at agreed timescales, ensuring information is accurately recorded and shared with residents
  • Maintain a visible presence at general needs and independent living schemes, signposting and referring residents for additional support and working in partnership with other teams to ensure the best possible outcomes
  • Promote and encourage social activities for residents, working with social committees, elected residents and local charities, and linking with the Customer Experience and Tenant Engagement teams
  • Provide rehousing advice and support for residents with changing needs, including guidance on the bidding process and direct liaison with CBL and West Suffolk Council as required
  • Work collaboratively across all teams to identify solutions, resolve resident queries efficiently and empower residents to manage their tenancies and report repairs independently

Requirements

  • A good understanding and knowledge of social housing, particularly supported housing, and the challenges and opportunities it presents for residents
  • A solid understanding of safeguarding and social care, and the ability to identify, record and refer concerns appropriately
  • Experience of liaising effectively with partner agencies such as social care, health services and voluntary organisations to achieve the best outcomes for residents
  • The ability to communicate with residents through a variety of methods, adapting their approach to work in a supportive, empathetic and person-centred way
  • A genuine commitment to working in a person-centred manner, placing the needs, wellbeing and independence of residents at the heart of everything they do
  • A full UK driving licence and access to their own vehicle
