Support Worker – Integrated Mental Health Service (IMHS)

The Integrated Mental Health Service (IMHS), in partnership with the East London Foundation Trust, welcomes ambitious individuals to drive meaningful change for Hackney residents. Play a leadership role in delivering early intervention, prevention, and support.
About the Role
Join our multidisciplinary IMHS teams as a Support Worker in this transformative Section 75 partnership. Your dedication will empower service users to maximise their independence and reduce dependency on statutory services.
Responsibilities
As a Support Worker, you will:
- Provide practical, emotional, and recovery-focused support to adults with severe and enduring mental health needs in the community.
- Collaborate with service users to achieve goals outlined in their support and care plans, fostering independence and wellbeing.
- Offer expertise in:
- Managing accommodation
- Budgeting and finances
- Daily living tasks
- Accessing community resources
- Deliver short-term enablement programmes, including:
- Hospital discharge assistance (“Home from Hospital”)
- Crisis prevention strategies (“Step-Up” intervention)
- Work closely with:
- Mental health professionals
- Carers
- Housing providers
- Health services
- Voluntary sector organisations
- Monitor and prioritise wellbeing risks, escalating safeguarding concerns while ensuring effective risk mitigation.
- Maintain comprehensive records and regular outcome reviews, aligning with the Care Act 2014 and recovery-oriented practices.
- Directly support service users in accessing opportunities for education, training, employment, leisure, and community participation.

Requirements
Ideal candidates will bring:
- Experience working with adults with mental health challenges in health or social care environments.
- Proven knowledge of preventative, enabling, and recovery-focused approaches.
- Strong interpersonal and organisational skills, with a commitment to building trusting relationships.
- A relevant vocational qualification in health or social care (preferred).
- Valued attribute: Enhanced DBS check (mandatory for this role).
