Support Worker - Community Based

Support Worker – Community & Home-Based Support (Warwickshire) We are looking for caring, reliable, and flexible Support Workers to join our growing team, providing person-centred support to individuals within their own homes and local communities across North and South Warwickshire. This is a rewarding role supporting people to achieve their personal goals, maintain their wellbeing and live the lives they choose. You’ll have the opportunity to build meaningful, long-term relationships, promote choice and dignity, and make a genuine difference to people’s daily lives Great support comes from consistency, trust and genuine connection. That’s why we aim to match support workers with people whose personalities, interests, and support needs are a good fit, allowing positive relationships to develop over time. Key Responsibilities Providing personal care and support with daily living activities. Supporting individuals with medication administration and management. Assisting with community access, appointments, social activities, and hobbies. Encouraging independence and promoting positive outcomes. Maintaining accurate records and reporting any concerns appropriately. Working in line with care plans, risk assessments, and organisational policies. What We're Looking For A full UK driving licence and access to a vehicle. Experience supporting individuals in a variety of care settings, with a minimum of 12 months of experience. Genuine passion for building positive relationships and helping people achieve their goals. Competence and confidence in administering medication. Experience providing personal care. Excellent communication and interpersonal skills. A flexible approach to working across different locations and support packages The ability to provide consistent support and develop trusting relationships with the people you support. A commitment to delivering person-led support that focuses on individuals' choices, independence and well-being. Working Pattern Availability to work a variety of shifts across the week, including weekdays and weekends. Opportunities for sleep-in shifts may also be offered.
