Support Worker – Dementia & Nursing Care

As a Support Worker, you’ll play a vital role in enriching the lives of our residents, delivering compassionate, high-quality care and supporting them to live as independently and fulfillingly as possible.

Responsibilities
As part of our supportive care team, your responsibilities will include:
- Providing high-quality, person-centred care and emotional support to residents with dementia and nursing care needs.
- Encouraging residents' independence while respecting their individual choices, preferences, and dignity.
- Supporting residents with:
- Personal care
- Daily living activities
- Meaningful social engagement
- Reinforcing existing life skills and promoting confidence and wellbeing.
- Acting as a Key Worker for designated residents, helping to:
- Plan
- Coordinate
- Review personalised care plans
- Developing positive working relationships with:
- Residents' families
- Healthcare professionals
- Other agencies involved in care
- Accurately maintaining care records and updating personalised care plans via our digital care system.
- Safely administering medication (assistance and administration requirements must align with company policies and training).
- Participating in:
- Resident meetings (promoting choice and inclusion)
- Regular supervision sessions
- Team meetings
- Ongoing training for professional development
- Working flexibly to meet the needs of our residents and contribute positively to the collaborative team environment.
- Upholding Hatzfeld Care Ltd’s values, policies, and commitment to outstanding care.

- Excellent communication and interpersonal skills, enabling the formation of meaningful relationships with residents, families, and colleagues.

- A compassionate, patient, and person-centred approach to care.
Preferable Qualifications:
- Previous experience supporting individuals with mental health conditions and/or dementia is desirable but not essential. Full training and support will be provided for the right candidate.
