Key Responsibilities
- Deliver Tailored Support: Provide responsive, person-centred assistance to individuals with complex needs, helping them achieve positive outcomes and independence.
- Foster Collaboration: Build open and trusting relationships with customers, colleagues, and partner agencies to ensure seamless and comprehensive support.
- Promote Participation: Encourage individuals to engage in social, educational and employment opportunities, preventing isolation and promoting self-determination.
- Manage Risks Effectively: Conduct thorough risk assessments and maintain professional boundaries, ensuring health, safety and compliance with safeguarding policies.
This role is being offered on a part time, fixed term/secondment basis working 22.2 hours per week until January 2027. You will be working 3 days per week, on a rolling 4 week rota, Monday to Sunday between 8am and 9pm, to include bank holidays, weekends. This role will be based at our Stoke House service in Penryn. Please note, this role will involve lone working.

- Have an understanding of support needs and support planning.
- Have experience of risk assessing and risk management.
- Be a great communicator with the ability to build rapport.
- Have accurate record keeping and computer skills.

Please note: This role is not eligible for a Cos. Applicants must have the right to work in the UK for the full duration of the role without a CoS.
