Social Work Assistant – Adults with Incapacity Case Worker

About the Role
This permanent, 35-hour per week position is based at Stonehouse Hospital (Salary: £37,702 – £38,779).
Key Responsibilities
The ideal candidate will:
- Coordinate active Adults with Incapacity (AWI) cases, ensuring compliance with procedures and best practice to minimise delays in discharges.
- Maintain effective communication with families, carers, and private law firms.
- Support the Hospital Discharge Mental Health Officer (MHO) in discharge planning.
- Conduct holistic risk and needs assessments, develop care plans, and review existing plans under supervision of the Operations Manager.
- Provide assistance to the Operations Manager in ensuring smooth operation of NHSGGC hospital discharge social work activity.
- Manage records across multiple platforms, including Liquid Logic and Microsoft Excel.
- Participate in duty rota coverage, supporting the Operations Manager.
- Work collaboratively with a cross-site caseload but remain aligned with the Operations Manager for support.
Proactively support individuals aged 16+ with diverse health and social care needs whilst upholding principles of the Social Work (Scotland) Act 1968.
Person Specification
- A highly motivated, organised, and flexible individual able to progressively enhance skills inCapacity casework.
- Demonstrable experience in working with vulnerable groups in a community or hospital setting.
- Excellent communication and administrative skills.
- Ability to work under pressure, adhere to deadlines, and maintain accurate records.
