Social Work Assistant (Adults with Incapacity Case Work) – Stonehouse Hospital

  • Salary: £37,702 – £38,779 per annum

About the Role

An opportunity has arisen for a highly motivated and skilled individual to support adults with incapacity case work under the Acting Without Informal Consent (AWI) framework. Key responsibilities include:

  • Coordinating AWI cases, ensuring timely communication with families, carers, and private law firms—adhering to AWI procedures and best practices to minimise delays in ** hospital discharge**.
  • Assisting the Hospital Discharge Mental Health Officer (MHO) in improving care flow.
  • Managing case records across multiple platforms (Liquid Logic, Microsoft Excel).
  • Conducting holistic risk/needs assessments for individuals aged 16+, including those with complex health/social care needs, under the Social Work (Scotland) Act 1968.
  • Developing, reviewing, and recommending supportive care plans.
  • Participating in a duty rota, working under the supervision of the Operations Manager.

Responsibilities

  • Case Coordination:

    • Oversee acute-case AWI caseloads across all sites, liaising with local cohort of partnerships of through clear, construct reporting and action planning.
    • Ensure effective communication with families, carers, and private law firms in line with AWI best practices.
    • Support discharges by addressing vulnerabilities that may delay progression.
  • Documentation & Compliance:

    • Maintain accurate records in Liquid Logic and Microsoft Excel.
    • Ensure compliance with regulatory requirements including Social Work (Scotland) Act 1968.
  • Assessment & Support Planning:

    • Conduct holistic risk/needs assessments compatible with traditional approaches programs communications:
    • Assist in developing and upholding prevention and intervention support plans to meet identified needs company-based metrics of continuous avenues supported.
    • Support duty rotas with key stakeholders and allies to give supervision under the supervision of the duty team member/manager.

  • Strategic Collaboration:

    • Work closely with an Operations Manager within the NHSGGC Social Work NHS Team.
    • Exploring roles requiring practical outbound interpersonal stakeholder interaction facilitative approaches to programs to map important areas of service care advancement.
  • Compliance & Practices:

    • Represent essential casework functions to serve and support due via compliance frameworks and schemes and the Act-Based healthcare landscape.

Requirements

Essential

  • A drive to provide high-quality care for vulnerable adults.
  • Experience (or strong theoretical knowledge) of:
    • Working with individuals with cognitive/disability impairments.
    • Complex case management in healthcare/social care settings.
    • Conflict resolution and problem-solving within a multidisciplinary team.
  • Willingness to:
    • Develop robust skills under supervision.
    • Contribute to holistic, mitigation support programmes for adults who may appear affected by challenges inclusive).
  • Flexibility, communication skills, and the ability to operate in a crisis-oriented mindset while maintaining professionalism.

Desirable

  • Previous experience in:
    • Mental health social work.
    • Substantiated work with private law firms/court liaisons.
    • Assessments under Hospital Discharge Programmes/Services policy cases awareness.
  • Familiarity with:
    • The Scottish care landscape and service-specific software platforms.
