Location: Lanark Health Centre Contract: Permanent Hours: 35 hours per week Salary: £52,392 – £53,177 per annum
About the Role
This position supports the Community Mental Health Team (CMHT) in fulfilling statutory obligations under:
- The Mental Health (Care and Treatment) (Scotland) Act 2003
- Adults Support and Protection (Scotland) Act 2007
- Adults with Incapacity (Scotland) Act 2000
The holder will:
- Manage high-tariff cases (civil and forensic) under the relevant legislation.
- Work with individuals requiring protection plans or complex case management.
- Collaborate with adults, older people, and young adults transitioning from children’s mental health services to adult services.
- Develop care plans that encourage service user participation, independence, and self-development.
- Collaborate with a multi-disciplinary team (e.g., healthcare, social work, law enforcement) to achieve positive outcomes.
- Ensure accurate record-keeping to reflect clients’ current needs and support plans.
