About The Role

Your primary responsibility as Team Leader will be to support the home’s management in delivering high-quality, person-centred care while upholding a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Key Responsibilities

  • Care Planning Leadership
    • Implement, monitor, and review person-centred care plans, including monthly evaluations.
    • Conduct care plan audits, develop action plans, and ensure timelines are met.

  • Resident Transitions

    • Streamline discharges by coordinating documentation and medication handover.
    • Ensuring next of kin are fully informed when necessary.
  • Resident Wellbeing

    • Monitor the well-being of each resident, noticing changes in care needs.
    • Adapting care plans and maintaining accurate, updated records.
  • Safety & Compliance

    • Promote safe systems of work, including rigorous risk assessments for resident moving/handling.
    • Prioritise personal and resident safety, following best practice protocols.

About You

To succeed, you will embody our values: caring, supportive, honest, respectful, and accountable in all actions.

Essential Criteria

  • Qualification: NVQ 3 in Care (or equivalent qualification) for care homes.
  • Experience: Two years’ experience in a senior role within residential care settings.
  • Communication: Strong ability to communicate effectively, both verbally and in writing.
  • Leadership: Demonstrated capability to lead staff and act responsibly in the manager’s absence.
