About The Role
Your primary responsibility as Team Leader will be to support the home’s management in delivering high-quality, person-centred care while upholding a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.
Key Responsibilities
- Care Planning Leadership
- Implement, monitor, and review person-centred care plans, including monthly evaluations.
- Conduct care plan audits, develop action plans, and ensure timelines are met.

Resident Transitions
- Streamline discharges by coordinating documentation and medication handover.
- Ensuring next of kin are fully informed when necessary.

Resident Wellbeing
- Monitor the well-being of each resident, noticing changes in care needs.
- Adapting care plans and maintaining accurate, updated records.

Safety & Compliance
- Promote safe systems of work, including rigorous risk assessments for resident moving/handling.
- Prioritise personal and resident safety, following best practice protocols.
About You
To succeed, you will embody our values: caring, supportive, honest, respectful, and accountable in all actions.
Essential Criteria
- Qualification: NVQ 3 in Care (or equivalent qualification) for care homes.
- Experience: Two years’ experience in a senior role within residential care settings.
- Communication: Strong ability to communicate effectively, both verbally and in writing.
- Leadership: Demonstrated capability to lead staff and act responsibly in the manager’s absence.
