We’re now looking for a warm, enthusiastic, and dedicated individual to join our award-winning team as a Team Leader at Wandsworth Common Care Home in London. If this sounds like the place for you, we’d love to hear from you!


About The Role

Your focus as a Team Leader will be to support the home management team in delivering high-quality care by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

Other Key Responsibilities:

  • Leading in the person-centred care planning process by:
    • Implementing, monitoring, and undertaking monthly evaluations
    • Completing care plan audits
    • Developing action plans
    • Ensuring action plans are completed

  • Assisting in ensuring a smooth transition for any discharges from the home, including:

    • Updating documentation and records
    • Managing medication handover processes
    • Keeping the residents' next of kin fully informed
  • Leading on monitoring the well-being of each resident by:

    • Ensuring awareness of any changes impacting care and care planning
    • Meeting changing needs and amending written records accordingly
  • Practicing safe systems of work across tasks, including:

    • Assessing risks linked to the moving and handling of residents
    • Ensuring personal safety and the safety of residents, visitors, and staff

About You

To succeed in your application, you will embody Avery’s values: caring, supportive, honest, respectful, and accountable.

Our Ideal Candidate Must:

  • Hold an NVQ 3 or equivalent in care for care homes
  • Have two years’ experience working in a senior position with the resident group
  • Demonstrate ability to communicate effectively both verbally and in writing
  • Show the capability to lead staff and act responsibly in the absence of the manager
