Role Overview
We are seeking an enthusiastic and motivated Band 7 Team Lead with hospital or community experience to lead our Integrated Acute Team, which is part of Manchester Children's Community Nursing Service (CCNS). If you have the skill set and drive to lead a large, skilled multidisciplinary team of staff delivering specialist nursing care to children with acute and chronic health needs, then this could be the job for you.
This is an exciting opportunity for a qualified children's nurse ready for their next career step into leadership. You will manage the Integrated Acute Team which incorporates acute, hospital at home, acute children's treatment, asthma and phlebotomy teams, in partnership with your Band 7 Team Lead colleague.
Key Responsibilities
- Play a key role in ensuring safe, effective, and compassionate care to children and young people in the local community and ensure pathways from secondary to primary care, to reduce and prevent hospital attendance, are followed.
- Provide management support, as required for cover purposes, to a variety of other CCNS teams such as Complex Care, Special Needs Nursing, Palliative Care, etc., ensuring high-quality, child-centred care to children and young people in their homes, schools, and community settings.

Main Duties
- Provide day-to-day line management of the Integrated Acute Team focusing on children with acute and chronic health needs, while working closely with the CCNS Matron.
- Take a leadership role in representing the CCNS at relevant meetings, including deputising where necessary for the CCNS Matron.
- Act as the lead clinician, together with your Band 7 colleague, for the Integrated Acute Team.
- Ensure a high standard of care for each child, working with all disciplines and agencies involved with the family.
- Communicate with partner agencies, parents, and children in an appropriate manner, often with sensitive information regarding diagnosis and outcomes.
- Work closely with RMCH, North Manchester General Hospital, and Wythenshawe Hospital to prevent inappropriate admissions/attendance.
- Provide clinical caseload oversight to a high professional standard using highly developed specialist knowledge and practical experience gained in a hospital or community setting.
