Team Manager – ASC Hub Outreach Team | BCP Council
Starting salary: £53,460 (pro rata for part-time colleagues)

Role Purpose
Mission: Help shape the future of preventative Adult Social Care in Bournemouth, Christchurch and Poole.
The ASC Hub is the central contact point for Adult Social Care (ASC) in BCP Council, integrating Contact, Outreach, and Intake services. The Outreach Team works proactively to engage individuals who are unfamiliar with or closed to receiving ASC support, visiting them in homeliness or community settings. The focus is on understanding what matters to them, identifying risks, and agreeing practical, outcome-focused solutions.
This team operates under a preventative, strengths-based model, aiming to:
- Advance independence
- Lower unnecessary long-term care demands
- Provide information and advice
- Support carers assessments
- Address housing needs
- Recommend low-level occupational therapy equipment
- Guide on care technologies
- Collaborate with housing, health, and voluntary sector partners
- Deploy qualified Occupational Therapists for targeted interventions
The Position
We are seeking an experienced, motivated Team Manager to lead our ASC Hub Outreach Team. This is a key leadership role focused on improving access to timely support, reinforcing prevention, independence, and early intervention.
Your responsibility will be to manage the Outreach Officers, ensuring they assist:
- Residents and carers
- Internal Public Service partners (e.g., health and housing)
- Externally associated organizations (non-profits, community spaces)
The outreach model aims to secure practical solutions, ranging from in-home assessments to occupational therapy advice, that help maintain residents' safety, well-being, and independence.
Impact: You will help design preventative strategies – branding a philosophy that reduces long-term care dependency, enabling independence earlier in a person’s journey. Leadership in promoting confidence-driven, strengths-based assessments and proportional, risk-informed care solutions is instrumental in ensuring positive resident-attained outcomes.

Main Responsibilities
Leadership and Staff Support
- Provide visible, hands-on leadership, combining day-to-day management and reflective supervision for the Outreach Team.
- Establish clear performance expectations, promote documentation, and reinforce consistent standards of multidisciplinary professionalism.
- Foster an environment where staff employ creative problem-solving to enhance outcomes through person-centred, strengths-based priority setting.
Cross-Collaborative Work
- Work tellingly with AS colleagues in Health, Housing, and the Voluntary and Community sectors to align support resources in a way that avoids excessive healthcare escalation.
- Represent the team in service development and improvement projects, ensuring the ASC Hub’s preventative model remains efficient and targeted.
Administrative and Professional Standards
- Lead in service improvement and monitoring to clarify efficacy, consistency, and standards within:
- Technology-equipment advising
- Practical assisted living interventions
- Uphold strict safeguarding awareness protocols, fostering Fair Decision-Making and ethical governance with person-centred approaches.
Listed responsibilities also encompass Organisational Leadership – ensuring change is handled effectively within a dynamic environment while upholding the team’s capacity to deliver excellence in person-centred interventions.
Ideal Candidate Requirements
Qualification & Registration
- Professionally qualified and registered with Social Work England (or an equivalent equivalent registration authority), e.g., HCPC (Health & Care Professions Council) or Nursing & Midwifery Council.
