Job Introduction

We are seeking an experienced and motivated Team Manager with a strong social work background to lead our Assertive Outreach Team. We are looking for a manager who can bring a clear social work perspective to a fully integrated health and social care service, ensuring that social work values, strengths-based practice, safeguarding, human rights, and statutory responsibilities remain central to service delivery. You will be working in an integrated team representing Bradford Council working alongside the Bradford District Care NHS Foundation Trust.

The team brings together professionals from Bradford Council and Bradford District Care NHS Foundation Trust within a single integrated service. You will work collaboratively to provide coordinated and holistic support for people who are often difficult to engage due to the complexity of their mental health needs with psychosis, and people experiencing social issues such as homelessness, substance misuse, and other forms of social exclusion. The successful candidate will provide professional social work leadership and management oversight, ensuring that individuals receive seamless, person-centred support through integrated care planning and intervention.

We are looking for a leader with excellent interpersonal and management skills who is passionate about supporting practitioners to deliver high-quality interventions with individuals experiencing multiple disadvantages. You will be confident in managing complex risk, providing professional guidance and oversight, supporting decision-making, and promoting a culture of positive risk enablement and strengths-based practice.

You will provide leadership, ensuring the effective delivery of statutory responsibilities. You will oversee complex casework, support staff in managing risk and safeguarding concerns, promote continuous professional development through reflective supervision and ensure the delivery of safe and effective practice that achieves positive outcomes for people with complex mental health needs.
