An exciting opportunity has arisen for a dynamic and experienced clinician to join the Integrated Discharge Team (IDT) as a Therapy Team Lead, supporting the delivery of safe, timely, and effective patient discharges across acute and community pathways.
The post holder will play a pivotal role in coordinating complex discharge planning within a multidisciplinary environment, ensuring person-centred care and optimising patient flow across urgent and emergency care systems. Working closely with acute teams, community services, local authorities, and system partners, they will lead on facilitating seamless transitions of care, reducing length of stay, and preventing avoidable delays.
The role requires advanced clinical reasoning and leadership to manage patients with complex needs, including frailty, multi-morbidity, and social care requirements. The successful candidate will proactively identify and resolve barriers to discharge, supporting the delivery of national discharge standards and models such as Discharge to Assess (D2A).
In addition to clinical responsibilities, the post holder will provide leadership, supervision, and support to junior staff, contribute to service development and quality improvement initiatives, and help drive performance against key operational metrics. Strong communication, decision-making, and system-working skills are essential to succeed in this fast-paced, high-impact role.
- Lead and coordinate complex discharge planning for patients with multifactorial needs, ensuring safe, timely, and person-centred transitions of care
- Work collaboratively within the Transfer of Care Hub and wider MDT to facilitate effective patient flow and reduce length of stay
- Act as a key point of contact between acute services, community teams, local authorities, and external providers, ensuring seamless communication and care coordination
- Proactively identify and resolve barriers to discharge, utilising escalation processes where required to prevent delays
- Support the delivery of national and local discharge standards, including Discharge to Assess (D2A) pathways and same-day discharge principles
- Use advanced clinical reasoning to assess patients with complex physical, cognitive, and social care needs, including frailty and multi-morbidity
- Provide leadership, supervision, and support to junior staff and therapy colleagues, promoting a culture of continuous learning and professional development
- Contribute to service development, quality improvement initiatives, and pathway redesign to enhance patient outcomes and operational performance
- Monitor and support performance against key metrics related to discharge, flow, and system pressures, taking action to address gaps where identified
- Maintain accurate and timely documentation, ensuring compliance with organisational policies and professional standards
