A fantastic opportunity has arisen for an enthusiastic, highly motivated and dynamic nurse to be a part of the Milton Keynes Community Tissue Viability team. We are looking to recruit a Tissue viability nurse to work as part of the team, providing complex wound care, education and advice across the Milton Keynes health economy. We are hoping to hear from individuals with a strong clinical background in wound care, with direct tissue viability experience.
The role will also involve providing guidance and training to nurses and health care professionals both in the inpatient units / nursing homes as well as in the community. The post holder will be expected to provide written advice and care plans to be implemented by staff caring for patients. They will also work closely with other specialist teams/disciplines to ensure a seamless service for our patients.
This role is part time, 9-5 four days a week (or hours spread over 5 days- can be negotiated). No unsocial hours or bank holidays.
Main duties of the job
CNWL is looking for a Tissue Viability Nurse Specialist to support the Milton Keynes Community Services and to provide Tissue viability support across Diggory division, including Mental Health Services and prisons. The main remit of the role will be to deliver complex wound care and formal and informal education packages, supporting healthcare professionals across the health care economy with advice and guidance. Completing assessments/staffing the complex wound clinic. Providing care in a clinic environment, patient’s homes, inpatient facilities including mental health units, prison and nursing/residential homes across Milton Keynes community.
The post holder will provide written advice and care plans to be implemented by staff in caring for patients. He/she will work closely with other specialist teams/Multi Disciplinary Team to ensure a seamless service to our patients.

Detailed Job Description And Main Responsibilities
The post holder will assist the Tissue Viability team in managing a clinical caseload which will be community based and attached both physical and mental health patients with Tissue Viability needs, to ensure delivery of cost and clinically effective tissue viability service to patients, carers and health care professionals.
Patients will be seen in a variety of settings, home, inpatient facilities, and within a clinic setting.
Main responsibilities:
- Act as a role model in expert practice facilitating and supporting the delivery of effective care by use of clinical judgement and decision making skills.
- Have responsibility for the assessment of care needs and the development, implementation and evaluation of care programmes both simple and complex for and with individuals and their families.
- Identify individual and collective tissue viability needs through observation, assessment and discussion such as Doppler studies, advanced Doppler studies and advanced wound assessment applications.
- Analyse and interpret collected information and collate as part of holistic assessment to identify patients’ needs, aetiology of wound, prognosis and then facilitate management programmes to address patient need.
- Assist the Tissue Viability team in managing complicated aspects in relation to tissue viability.
- Liaise with other organisations, Voluntary, Statuary and Health, to provide holistic care.
- Co-ordinates care for individuals and their carers
- Engages patients and their carers in all aspects of care planning and delivery
- Implements the framework of informed consent
- Provides health advice and support for other agencies.
- Work with patients, carers and families to enable them to develop self-care skills with respect to health and health services
- Document consultations and practice to incorporate both audit and the setting, and monitoring and evaluation of standards of care.
- Promoting the delivery of seamless services
- Implements the National Service Frameworks and NICE guidance.
- Work with other partners to facilitate access to other programmes to promote health for the community (e.g. ICT training, basic skills training and adult education).
- Uses quality standards in all aspects of care planning and delivery
- Prescribes and Implements evidence based practice at all times, including (TNP) VAC, bio-surgical debridement, and advanced wound therapies.
- Plans and implements management of chronic diseases and works towards preventing the complications of long term conditions.
- Provides formal and informal training across the health economy.
- Assists with promoting Pressure ulcer prevention/agenda including management of Datix reporting and Root cause Analysis investigations.
- Provides advice on pressure reducing/relieving equipment.
- Contribute to Quality improvement projects across the service.
