Are you an experienced and motivated health professional looking for an opportunity to make a real difference in preventing unnecessary hospital admissions and supporting patients to receive the right care, at the right time, in the right place?

We are seeking 2 x Band 6 professionals to join our multidisciplinary team, providing high-quality clinical triage and care coordination across urgent and community healthcare services. Working within a fast-paced environment, you will be responsible for assessing patients presenting with a wide range of health needs via telephone, digital platforms, and face-to-face contacts, ensuring they receive the right care, at the right time, in the right place.

LSCFT are seeking 2 x skilled Band 6 Triage Clinical Coordinators to join our Care Connexions team. This is an exciting opportunity to work at the heart of integrated care, playing a key role in the clinical triage, prioritisation, allocation, and coordination of referrals across a range of urgent and community-based services.

This is a rewarding and fast-paced role where no two days are the same. You will be instrumental in supporting patients to remain safely in their own homes, reducing avoidable hospital admissions, facilitating seamless care coordination, and improving patient outcomes. We are looking for clinicians with experience of working across health and social care settings, excellent communication and decision-making skills, and a strong understanding of integrated community services and admission avoidance pathways.
