Triage Mental Health Practitioner

Registered Mental Health Nurse – Community Mental Health Triage Team

Role Overview

The post holder will join the North Westminster and Queen’s Park & Paddington Community Mental Health Triage Teams, serving as the primary access point for mental health referrals across 22 GP practices. The role requires a Registered Mental Health Nurse to deliver accessible, high-quality care within integrated community settings.

Key Purpose

The successful candidate will:

  • Conduct mental health and risk assessments
  • Manage referrals and provide signposting
  • Offer short-term interventions in line with patient needs
  • Support continuity of care between primary and community services
  • Work collaboratively with GP practices, MDTs, and external agencies

Main Responsibilities

Core Duties

  • Triage and assessment:

    • Receive and triage GP referrals, completing comprehensive psychosocial assessments to identify risks, needs, and appropriate care pathways.
    • Screen routine and enhanced referrals, determining suitability for primary care management based on thresholds and resource availability.
  • Clinical support and liaison:

    • Act as a key liaison for GP practices, supporting complex cases and co-morbidities.
    • Facilitate timely onward referrals to specialist teams, mental health hubs, or external agencies.
    • Ensure early identification and fast-tracking of high-risk or severe mental health cases.
  • Safeguarding and intervention:

    • Respond to safeguarding concerns with timely escalation and adherence to procedures.
    • Refer patients to psychological therapies or community support as appropriate.
    • Provide brief, evidence-based interventions in alignment with patient needs.

Person Specification

Qualifications

Essential:

  • Current Professional Qualification in mental health nursing, social work, or occupational therapy.
  • Live registration with the relevant professional body.
  • Evidence of post-registration training and Continuous Professional Development (CPD).

Desirable:

  • Training in psychosocial interventions.

Experience

Essential:

  • Experience working in a multidisciplinary mental health team with clients with acute, severe, or enduring mental health problems.
  • Extensive post-registration experience in the field.

Desirable:

  • Work with clients experiencing homelessness or rootless situations.
  • Hands-on experience with mental health crises, including crisis intervention.
  • Experience in supervising students or junior staff, audit, research, or substance use services.
  • Teaching or mentoring experience.

Skills and Knowledge

Essential:

  • Exceptional verbal and written communication skills, including report writing and IT proficiency.
  • Ability to assess, plan, implement, and evaluate care packages using diverse resources.
  • Experience in psychosocial intervention frameworks.
  • Mastery of risk assessment and management.
  • Understanding of psychosis and its impact on service users and carers.
  • Knowledge of current legislation and guidance relating to mental health.
  • Cultural competence with diverse populations, including inner-city and Black and minority ethnic (BME) groups.
  • Ability to engage with marginalised communities effectively.

Desirable:

  • Leadership skills, including motivation, initiative, and change management.
  • Basic counselling and group work skills.Support the Hub depot clinic as required under a shared rota system. Experience in working with complex cases where mental health overlaps with substance misuse.
