About the Role

The PEEER Youth Service is a respected health-based youth work provision dedicated to supporting young people (aged 11–25) living with a long-term health condition, particularly those with kidney-related conditions, across the Wessex Region.

Core Responsibilities

This experienced Youth Worker will:

  • Provide targeted support to young people managing long-term kidney conditions, promoting their emotional well-being, social development, and healthcare access.
  • Work within a healthcare setting, especially alongside the paediatric nephrology team, bridging support needs before, during, and after hospital-based treatments.
  • Deliver a varied programme of activities, including online and in-person interventions, emotional wellbeing groups, community projects, and one-to-one youth work, both in clinical and community settings.

The role emphasises:

  • A youth-led approach—collaborating with young people to shape programmes that meet their needs.
  • Transition support as patients move from paediatric to adult healthcare, ensuring continuity and best-practice collaboration between teams.
  • Flexibility to navigate complex needs, from support during treatments and procedures to navigating education, social isolation, and aspirational developmental challenges.

Working with PEEER Youth Service

Detailed Job Description

Main Responsibilities

  • Support young people (11–25 years) with kidney conditions—emotionally, socially, and developmentally—to navigate health challenges.
  • Integrate as a core member of the healthcare team, collaborating with clinicians to ensure cohesive patient care.
  • Act as a social prescribing link worker, connecting young people with community resources (recreational, educational, advocacy) based on their individual needs.
  • Implement a diverse programme of engagement, tailored both collaboratively with the wider PEEER Youth Service team and independently where necessary.
  • Facilitate transition from paediatric to adult healthcare—using current best-practice guidelines and fostering strong inter-team relationships.
  • Work across hospital, clinical, and community settings, with flexibility in hours and travel requirements to support young people’s transitions.

Essential Criteria

Qualifications and Experience

  • A Professional Youth Work Qualification (or equivalent degree/diploma) in Youth Work.
  • Completed (and maintained) Continuing Professional Development reflective of the field.
  • Evident significant experience working with young people, preferably in health-related or social care settings.
  • Comprehensive understanding of:
    • Health needs of young people (especially chronic illnesses).
    • Social challenges (e.g., isolation, developmental gaps).
    • Child protection, cultural diversity, and intersectional issues.
    • Consent, confidentiality, and adolescent care questions.

Professional Competencies

  • Ability to support young people within clinical environments, including support during medical procedures.

Desirable but Not Essential

  • Previous experience in hospital or healthcare-based youth work.
  • Knowledge of community support services and their accessibility.
  • Familiarity with local authority policies and procedures.
  • Experience in facilitating groups or leading workshops.
