Community Staff Nurse
District Nursing Service – Fixed Term/Ssecondment Position
Job Overview
This post is fixed-term / secondment until 26.06.2027 due to maternity cover.

About the Opportunity
An exciting secondment and career development opportunity for experienced registered nurses with enthusiasm, vision, and a passion for making a difference. Our District Nursing Service is dynamic, proactive, and progressive, committed to delivering outstanding patient-centred care in the communities of Anglesey and Gwynedd.
We are seeking keen Band 5 Community Nurses who aspire to develop into one of the following specialist roles:
- Band 6 District Nurse (by undertaking the District Nurse Qualification)
- Chronic Condition Case Manager (via disease-specific training for complex care)
- Nurse Practitioner (through a modular clinical skills training programme)
Interested candidates are encouraged to contact the department for an informal discussion about requirements.
Main Duties & Key Responsibilities
The ideal candidate will:
- Demonstrate exceeding communication skills and the ability to deliver clinically effective care as part of a District Nurse Team.
- Work collaboratively with partner organisations to provide high-quality, evidence-based care, supporting patients to:
- Remain in their own homes and communities
- Avoid hospital admissions
- Develop self-management skills
- Access seamless patient-centred treatment
- Travel efficiently between patient homes (both urban and rural) in a timely manner.
- Speak Welsh (desirable but not essential).

Required Qualifications & Criteria
Essential
- Registered Nurse (or within 2 months of gaining Prepared to Practice (PIN)).
- Evidence of Primary Care knowledge.
Desirable
- Working towards a diploma or degree in nursing.
- Previous community nursing experience.
Skills
Essential
- Strong interpersonal and communication skills.
- Ability to maintain high standards of clinical care.
- Proven clinical competency.
Desirable
- A flexible approach to work responsibilities.
Knowledge
Essential
- Proof of ongoing professional development and training.
- Familiarity with Professional Codes of Conduct.
- Broad practical knowledge of:
- Community nursing competency requirements.
- Welsh health systems (advantageous for local roles).
Desirable
- Meets all desirable criteria outlined in the full job specification.
Additional Requirements
Essential
- Willingness to work unsociable hours.
- Ability to travel promptly between patients’ homes for timely care.
Desirable
- Getting Certified Speech/Broadcast Quality Welsh (Welsh Speaker).
