🌟 We’re Hiring: Diabetes Practitioner
Field-based position with travel across the Black Country Permanent (zero-hour contract) £12.48 per hour (administrative rate / £15.79 per one-to-one session), plus mileage reimbursement Hours: Tuesdays 12:00–18:00, Wednesdays 09:00–17:00, Fridays 09:00–17:00
🔍 About the Role
As a Diabetes Practitioner, you’ll play a vital role by supporting individuals enrolled in our Type 2 Diabetes Remission Programme. Over the course of a year-long programme, you’ll assist participants in:
- Reducing blood sugar levels
- Building confidence in adopting sustainable lifestyle changes
- Fostering long-term health engagement
Through personalised coaching, delivered in-person, by phone, or digitally, you’ll provide tailored support, guiding individuals through:
- Goal-setting for realistic, lasting health improvements
- Motivation and engagement strategies
- Challenges, progress tracking, and overcoming barriers
By fostering trust and a person-centred approach, you’ll empower service users toward remission and improved overall wellbeing.

🎯 Responsibilities
- Deliver personalised one-to-one health coaching sessions
- Empower service users through individualised behaviour and lifestyle change support
- Guide participants to set and achieve realistic health goals
- Monitor progress and promote continued engagement
- Build compassionate, trust-based relationships
- Collaborate with team members to deliver high-quality outcomes
✨ What You’ll Bring
We’re seeking candidates with one or more of the following:
- A minimum undergraduate degree in:
- Nutrition
- Health-related sciences (e.g., Sports, Exercise Science, Health Psychology)
- Related qualifications in personal training (Level 2/3)
- Experience in phone-based coaching
- Technological adeptness with Microsoft Office and healthcare systems
- Dedication to delivering evidence-based, person-led health support
🛡️ Experience and Qualifications Required
- A relevant undergraduate degree, diploma, or equivalent in:
- Nutrition, Dietetics, Health Psychology, or Sport Science
- Alternatively, a CIMSPA fitness qualification (Level 3 minimum)
