Senior Social Worker – Short Term Support Service

About the Role

This is an incredibly exciting time to join our service. We have launched Living Better Lives in Lancashire, our new strengths-based vision for Adult Social Care. Through this initiative, we are embedding innovative practice that will make a real difference to the lives of people in our communities.

We are seeking Senior Social Workers to support us in delivering our vision: supporting people to achieve what matters to them, living independently in a place they call home.

Collaboration and co-production are at the heart of our work. You will partner with:

  • Health professionals
  • Care families
  • Community organisations
  • Carers
  • Most importantly, the people we support

Together, we will shape meaningful and personalised outcomes.


Key Responsibilities

As a Senior Social Worker within the Short Term Support Service, your role will involve:

  • Facilitating safe, timely, and person-centred discharges from hospital settings.
  • Collaborating with health professionals, families, and partners to ensure individuals transition to the most appropriate care, promoting recovery, independence, and a positive discharge experience.
  • Managing a complex caseload, applying your professional expertise to make effective decisions and achieve high-quality outcomes.
  • Providing guidance and support to Social Workers while contributing to their development and ensuring consistent best practice across the team.

Hours & Additional Details Twelve-month contract (£37,618 - £41,721 p.a. Band 7 (SCP 15)). 37 hours per week, with work commitments including one in four weekends.
