About the Role
We are recruiting empathetic and proactive Social Workers who are passionate about creating meaningful change. You will help people live as independently as possible in their own homes by finding creative, supportive solutions.

As a Social Worker within the Older People & Physical Disabilities Service, your role will include:
- Conducting assessments to determine support needs.
- Developing personalised care plans for service users.
- Helping individuals access local services while maintaining dignity and independence.
- Working collaboratively with agencies, carers, and professionals to deliver holistic care.

Responsibilities
Your duties will be vital in shaping futures. You will:
- Carry out assessments and support planning to enable residents of Lancashire to live safely and independently.
- Promote health, wellbeing, and social inclusion while maintaining the person-centred approach.
- Collaborate with care teams, clinicians, and family members to tailor interventions.
- Engage in innovative practice to improve outcomes for service users.
- Continuous improvement – as part of the Living Better Lives in Lancashire initiative, you will contribute to pioneering social care solutions.
Requirements
- Qualified Social Worker: Must be **registered (or in the process of registering) with Social Work England (SWE).
- Empathy & Compassion: A genuine desire to support vulnerable individuals.
- Innovative Mindset: Willingness to develop fresh approaches in line with strength-based values.
- Personal access to a business-use vehicle (you must provide insurance coverage as part of this role).
🔍 DBS Check Required: This role is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975, requiring a Disclosure and Barring Service (DBS) check.
